Compound stress fracture: what the term means

Short answer
A compound fracture breaks the skin, while a stress fracture develops from repeated loading; the combined phrase needs clinical clarification.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Terminology
- Compound means open to the skin; stress describes how repeated loading injured bone.
- First response
- Cover an open wound, immobilise the limb and obtain emergency care.
- Activity
- Stop impact loading until a suspected stress injury has been assessed.
The short answer
Compound fracture is another name for an open fracture, where a wound communicates with the broken bone. A stress fracture is a small crack or bone stress injury caused by repeated load exceeding the bone's ability to recover. Stress fractures are usually discussed as open or closed, so the exact diagnosis matters.
How clinicians interpret the injury
A wound near a painful bone changes the situation because an open fracture can become contaminated and needs urgent treatment. Do not assume a small wound is superficial; the skin opening may be distant from the point of the break.
Without a wound, gradually increasing focal pain during running, marching or jumping is more consistent with a stress injury. Foot pain, shin pain, heel pain or thigh pain can have many causes, and examination plus imaging helps locate the problem. Early imaging can sometimes appear normal, so follow-up may be needed when suspicion remains.
What to do while awaiting assessment
Stop impact activity and avoid loading a limb that is painful to bear weight on. For a suspected open fracture, cover the wound with a clean dressing, keep the limb still and do not push exposed tissue or bone back inside. Do not eat or drink until emergency staff advise you, because a procedure may be required.
For a closed stress injury, treatment depends on the bone involved and its healing risk. A clinician may recommend protected weight bearing, activity modification and assessment of training load, nutrition and bone health. Vitamin D deficiency may be considered when risk factors are present; supplementation should match clinical advice rather than guesswork.
When to seek urgent care
Call emergency services for visible bone, heavy bleeding, a badly deformed limb, a pale or cold foot, loss of feeling, or inability to move the toes after injury. A wound over a suspected fracture needs urgent emergency assessment even when the opening looks small.
Seek prompt assessment for focal bone pain that persists at rest, worsening limping, swelling or inability to bear weight. Sudden severe pain after a period of activity-related discomfort can indicate that a stress injury has progressed to a complete fracture.
Questions that clarify the plan
Ask whether the fracture is open or closed, which bone and area are affected, and whether more imaging is needed. Confirm how much weight you may place through the limb, what support to use, which risk factors need checking and how return to work or sport will be staged.

Questions people ask
Can a stress fracture become a complete fracture?
Yes. Continued loading can allow a bone stress injury to progress. Increasing pain, pain at rest or sudden inability to bear weight should be assessed promptly.
Can an open fracture have only a small skin wound?
Yes. The opening can look minor and may not sit directly over the break. Any wound near a suspected fracture needs urgent evaluation for communication with the bone.
