Comedonecrosis in DCIS: what the finding means

Ductal carcinoma in situ, or DCIS, consists of abnormal cells confined inside breast milk ducts. Comedonecrosis means that cells have died in the centre of an involved duct and left debris. It is often reported with other features, including nuclear grade, lesion size, hormone-receptor findings, and whether abnormal cells reach the edge of removed tissue.

The finding can occur in higher-grade DCIS, but no single word in the report determines the entire treatment plan. A biopsy samples only part of an abnormal area, while tissue removed during surgery lets the pathologist assess it more fully and check for any invasive focus.

Review the mammogram and biopsy findings with a breast surgeon or multidisciplinary breast team. Surgery may involve lumpectomy or mastectomy, depending on the extent and distribution of DCIS, breast size, previous treatment, and personal preferences. Radiotherapy may be discussed after breast-conserving surgery. Hormone therapy may be considered when the abnormal cells have relevant hormone receptors.

Request a copy of the pathology report and note whether the diagnosis came from a needle biopsy or a surgical specimen. If surgery has already taken place, ask about margin status because this indicates whether DCIS is present at or near the cut edge.

Comedonecrosis itself does not create a medical emergency. Arrange clinical assessment for a new breast lump, bloody nipple discharge, nipple or skin retraction, or a persistent breast change. After a biopsy or operation, seek prompt advice for spreading redness, increasing swelling, pus, fever, heavy bleeding, worsening pain, or breathlessness.

Ask about the DCIS grade and extent, whether imaging and pathology findings agree, and whether more tissue needs evaluation. For planned surgery, discuss the chance of needing another procedure if margins contain DCIS, the expected cosmetic result, and how radiotherapy or hormone-receptor status may affect later choices.
