Cold hands and low blood pressure

Blood pressure can fall temporarily after standing, during illness, with dehydration, or as a side effect of some medicines. The body may narrow blood vessels in the skin, making hands feel cool while you feel light-headed or weak. Some people have a low reading without symptoms; the reading and how you feel both matter. Cold hands by themselves do not establish hypotension.

Sit or lie down when dizzy, rise slowly, and avoid driving or climbing until you feel steady. Keep warm and take fluids as normally advised for you. Do not add salt, stop medicine, or deliberately increase fluids without professional guidance, especially if you have heart or kidney disease. Note symptoms, posture, recent illness, food and drink, and medicine timing. Arrange a GP consultation when episodes repeat or interfere with daily activities.

Get urgent help if cold hands occur with fainting, chest pain, severe breathlessness, confusion, marked weakness, or signs of shock such as clammy skin. Seek prompt assessment for a sudden new episode, repeated falls, or a hand that becomes pale, blue, painful, or numb on one side.

Ask whether your blood pressure should be checked lying and standing, whether a medicine or illness could be contributing, and what readings or symptoms should trigger contact. Bring home readings if available, with the time and your position.
