Cold hands and heart failure

Heart failure means the heart is not pumping enough blood for the body’s needs. The body may redirect blood toward vital organs, leaving hands and feet cool. Heart failure more often presents with a combination such as breathlessness, fatigue, a cough, or swollen ankles. Atrial fibrillation and hypertension can contribute to heart strain, but cold hands may also reflect room temperature, anxiety, medicines, anaemia, or other circulation problems.

If you have known heart failure, record when your hands feel cold and whether this occurs with a change in breathing, activity, weight, swelling, or energy. Take prescribed medicines as directed and do not change fluid or salt advice without your care team. Contact your clinician if symptoms are new, persistent, or different from your usual pattern. A chronic disease review can bring together symptoms, examination, blood pressure, heart rhythm, and treatment needs.

Seek emergency help for severe or rapidly worsening breathlessness, chest discomfort, fainting, blue lips, new confusion, or inability to speak in full sentences. New coldness with marked weakness, collapse, or a pale or blue limb also needs urgent assessment. Do not wait for a routine appointment when several symptoms suddenly appear together.
