Cold and wet heart failure: what should you know?

In this clinical shorthand, wet refers to fluid building up because the heart is not moving blood effectively; this may cause breathlessness, cough or swollen ankles. Cold refers to reduced forward blood flow, which can be associated with cool hands and feet, fatigue, dizziness or confusion. This pattern can occur during worsening Heart Failure and is more concerning than stable symptoms. Hypertension and atrial fibrillation can contribute to heart strain or an irregular rhythm, but only an assessment can establish the cause of a change.

Follow the treatment plan you were given and take medicines as prescribed; do not change diuretics or restrict fluids unless your clinician has provided instructions. Weighing yourself consistently may help your care team notice a change, if they have recommended it. Keep a record of breathlessness, swelling, cough, fatigue, palpitations and how much activity you can manage. Arrange prompt review when these symptoms worsen or appear together.

Call emergency services for severe breathlessness at rest, chest pain, fainting, new confusion, blue lips, or coughing up frothy or blood-stained sputum. Seek urgent medical advice for rapidly increasing ankle swelling, a sudden change in weight, markedly reduced urine, worsening fatigue or a new irregular heartbeat. Do not drive yourself if you feel faint or severely breathless.
