Coat hypertension

Some people become tense, hurried or alert during a medical visit, which can temporarily raise blood pressure. A clinic reading may therefore not represent your usual level. However, a normal home reading does not automatically dismiss the clinic result: blood pressure can also rise outside the clinic and remain unnoticed. Repeated measurements, home monitoring or ambulatory monitoring can help distinguish coat hypertension from sustained hypertension. Long-term high blood pressure can affect the heart, brain and kidneys, so the pattern deserves follow-up.

Use a validated upper-arm monitor if your clinician recommends home checks. Rest quietly before measuring, keep your back supported and your arm at heart level, and avoid talking during the reading. Record the date, time, reading and any symptoms or relevant medicines. Bring the monitor and log to your appointment so technique and device accuracy can be checked. Discuss sleep problems, weight changes and medicines that may affect blood pressure, rather than changing treatment on your own.

Seek emergency help for a very high reading accompanied by chest pain, severe breathlessness, weakness on one side, trouble speaking, new confusion, fainting or a sudden severe headache. These symptoms can occur with serious problems such as stroke or heart complications. Without emergency symptoms, arrange follow-up if readings remain high, differ greatly between home and clinic, or you are taking blood-pressure medicine and feel dizzy.

Ask whether home or ambulatory monitoring is suitable, how often to measure, and what range should trigger a call. Ask whether other conditions, including obstructive sleep apnoea, obesity or kidney disease, could affect your readings.
