Coarse wheezing

Wheezing often arises when air moves through narrowed lower airways. A coarse or rattling quality may also occur when mucus is moving in the breathing tubes. Acute bronchitis can produce cough and noisy breathing; asthma and COPD can cause variable airway narrowing. Allergic rhinitis may worsen breathing symptoms, while bronchiolitis is more relevant in infants and young children. The sound alone cannot distinguish these conditions, and a clinician may need to listen to your chest and measure breathing.

Notice when the sound occurs, whether it follows exercise, a cold, smoke, dust or an allergen, and whether cough, fever, chest tightness or breathlessness is present. Avoid smoke and other known irritants, drink fluids if you can, and use prescribed inhalers exactly as directed. Do not borrow another person’s inhaler. If a clinician has recommended nebuliser treatment, follow the supplied instructions and seek advice if it is not relieving symptoms.

Call emergency services for severe or rapidly worsening breathlessness, blue or grey lips, inability to speak in full sentences, faintness, confusion, or swelling of the lips, tongue or throat. Sudden wheezing after food, medicine or an insect sting may be anaphylaxis. A new wheeze, wheeze with chest pain, or breathing difficulty in a baby also needs urgent medical assessment.

Ask what part of the airway is producing the sound, whether you need lung-function testing or a chest examination, and which warning signs should prompt urgent review. Ask how to use any inhaler and when to reassess if the wheeze persists.
