Cloudy, smelly pee without pain: what to know

Urine can look cloudy when it contains extra salts, mucus, cells or crystals. A strong smell may happen when urine is concentrated after drinking less fluid, or after certain foods, vitamins or medicines. A urinary tract infection can also present with cloudy or unpleasant-smelling urine, even when burning is absent. A kidney infection is more concerning when the change comes with fever, chills, pain in the side or back, nausea, or feeling markedly unwell. The appearance alone cannot identify the cause. Notice whether the colour and odour improve after you drink fluids regularly, unless a clinician has told you to restrict fluids. Do not use leftover antibiotics or someone else’s medicine. Keep track of how long the change lasts and whether you develop urgency, frequency, discharge, blood, fever or pain. If it persists, recurs, or is accompanied by urinary symptoms, arrange a medical review; a urine sample may help guide the next step.

Seek prompt medical care for fever or chills, pain in the back or side, vomiting, visible blood in the urine, confusion, or difficulty passing urine. These features can signal infection or another problem needing timely treatment. Arrange routine care if the cloudiness or smell continues despite normal hydration, or if you are pregnant, have kidney disease, or have a weakened immune system.
