What should you know about clinical gait analysis?

Short answer

Clinical gait analysis examines how you walk or run to identify movement patterns that may contribute to pain, instability or reduced function.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Purpose
Connect movement patterns with symptoms and functional limits.
Assessment
History, physical examination and observation, with measurement tools when useful.
Possible outcome
A plan for exercises, footwear, technique, orthotics or further evaluation.

The short answer

It combines your history and symptoms with a physical examination and repeated observation of your movement. Depending on the clinical question, the assessment may also measure joint motion, muscle activity, timing, balance or pressure beneath the feet.

What the assessment can show

A clinician looks at how the trunk, pelvis, hips, knees, ankles and feet work together through each part of a step. Findings may include asymmetry, limited joint movement, altered foot loading, poor balance or a compensation for pain or weakness. A single unusual feature does not establish a diagnosis; it must be interpreted alongside symptoms, strength, flexibility and relevant medical history.

Video can help replay movement, while motion capture can quantify joint positions and a pressure plate can map how weight moves across the feet. The equipment chosen should match the question being investigated rather than replace the clinical examination.

How to prepare and use the results

Bring shoes you regularly use and clothing that allows the clinician to see relevant joint movement. Explain where symptoms occur, what activities bring them on, whether they change with speed or distance, and any previous injury, surgery, brace or orthotic use. You may be asked to walk or run several times at a comfortable pace.

Results may guide a targeted exercise programme, footwear advice, technique changes, further examination or a trial of an assistive device or orthotic. Useful follow-up focuses on whether symptoms and function improve, not merely whether every measured angle changes.

When to seek medical care

Arrange an assessment if a persistent limp, recurrent falls, repeated joint pain or difficulty walking is limiting everyday activity. Seek urgent medical care for sudden inability to bear weight, a visibly deformed limb, new weakness or numbness, loss of bladder or bowel control, or a hot swollen joint with fever. These concerns need medical evaluation rather than gait testing alone.

Questions to ask your clinician

Ask which part of your gait appears linked to your symptoms, whether the finding could be a compensation, and what change would count as meaningful improvement. It is also useful to ask whether basic observation is sufficient, whether instrumented testing would alter management, and when your progress should be reassessed.

Questions people ask

Is clinical gait analysis only for runners?

No. It can assess walking or running in people with pain, balance problems, neurological conditions, injuries, joint disorders or difficulty using a prosthesis or brace.

Does an unusual gait pattern always need correction?

Not necessarily. Clinicians consider whether the pattern is connected to symptoms, safety or reduced function before recommending a change.

Will gait analysis give a diagnosis by itself?

Usually not. Its findings contribute to a broader assessment that may include strength, joint movement, neurological testing and imaging when clinically appropriate.
