Chronic kidney disease and gout

Gout occurs when urate crystals provoke inflammation in a joint. Impaired kidney handling of uric acid can contribute to higher blood urate, while dehydration and some medicines may also influence attacks.

Gout commonly presents with sudden severe joint pain, warmth, redness and swelling, often beginning in a single joint. These findings can resemble a joint infection, so a new hot swollen joint is not automatically gout even in someone who has had previous attacks.

Kidney function affects the choice and dose of medicines used for an acute attack and for long-term urate control. Some pain relievers can worsen kidney function or fluid retention, and a drug that was suitable previously may need review after kidney function changes.

Contact your usual clinician early in an attack for a kidney-appropriate treatment plan. Provide an up-to-date medicine list, including diuretics, non-prescription pain relievers and supplements. Do not start an anti-inflammatory medicine or change urate-lowering treatment without checking that it is suitable for your kidney function.

Take prescribed long-term therapy consistently, attend planned kidney and urate blood tests, and follow personalised fluid advice. Sudden stopping and restarting can make control harder. Dietary adjustments may support care, but they do not replace treatment when urate remains persistently elevated.

Seek urgent assessment for a hot swollen joint with fever, chills, marked illness, a wound near the joint or inability to bear weight, because joint infection must be considered. Prompt care is also needed for sharply reduced urine, breathlessness, confusion or rapidly increasing widespread swelling.

Ask which attack treatment is safest for your current kidney function, whether any medicine raises uric acid, what blood monitoring is planned, whether the swollen joint needs fluid sampling, and how to handle long-term gout medicine during a flare or intercurrent illness.
