CKD and blood pressure: what to know

Healthy kidneys help regulate fluid and salt balance and contribute to blood-pressure control. When kidney function is reduced, extra fluid or altered hormone signals may raise pressure. High pressure can also strain the small blood vessels that filter blood in the kidneys, causing further damage over time. The relationship can therefore run in both directions; having high blood pressure does not by itself prove that someone has chronic kidney disease.

A clinician may follow home or clinic readings alongside blood and urine tests. Results are interpreted together because a single reading, illness, or medication change may not reflect your usual pattern.

Take prescribed medicines as directed and do not stop them without advice. Keep a record of readings if your clinician has asked you to monitor at home, noting the time and any symptoms. Discuss salt intake, suitable activity, smoking, weight, and medicines such as anti-inflammatory painkillers, which may need special consideration when kidney function is reduced. Attend planned kidney and blood-pressure reviews so treatment can be adjusted safely.

Arrange prompt medical advice for a sustained change in readings, new swelling, much less urine, worsening breathlessness, or repeated vomiting. Seek emergency help for chest pressure, severe breathlessness, sudden weakness on one side, new trouble speaking, or a sudden severe headache. These symptoms can signal urgent problems such as Angina, Heart Failure, Stroke, or a Transient Ischaemic Attack and need immediate assessment.

Ask which blood-pressure range is appropriate for you, how often kidney blood and urine tests are needed, whether any current medicines require a review, and which home readings should trigger a call. You can also ask how sleep problems, weight, or other conditions may affect control.
