What should you know about cis bladder cancer?

CIS means carcinoma in situ. Although it has not grown into the bladder muscle, its high-grade cells can behave aggressively, so the care team will discuss active treatment rather than simple observation.

CIS grows as a thin, flat area along the urothelium, the tissue lining the inside of the bladder. It may occur alone or beside another bladder tumour. Because it does not necessarily form a raised mass, assessment may combine cystoscopy, urine cytology and biopsies. The pathology report confirms the cell type and grade; the wider assessment checks whether disease is present elsewhere in the urinary lining.

Review the pathology result with a urologist and clarify whether CIS was found alone or with another tumour. Treatment commonly places medicine directly into the bladder through a catheter; intravesical BCG is one option. Further choices depend on previous treatment, response, other bladder findings and general health. Persistent or returning high-grade disease may lead to discussion of other bladder medicines, systemic immunotherapy or bladder removal surgery. Planned cystoscopies, urine tests and sometimes repeat biopsies are essential because a clear scan or improved symptoms cannot confirm that CIS has gone.

Contact the treating team promptly for new visible blood in the urine, worsening burning, fever, chills or increasing pelvic pain, especially during treatment. Seek urgent medical care if you cannot pass urine, blood clots appear to block the flow, bleeding is heavy, or you feel faint or acutely unwell. These features need assessment for obstruction, bleeding or infection; they do not by themselves show whether CIS has progressed.

Ask whether the biopsy shows CIS only or another bladder tumour as well, whether all relevant areas were sampled, and what treatment is recommended first. It is also useful to ask how response will be checked, what side effects should trigger a call, what happens if CIS remains after treatment, and how the proposed plan may affect urination and daily routines.
