Symptoms at a glance

Symptoms may arrive as distinct flares or persist at a lower level between attacks. The feet and ankles are familiar sites, but knees, hands, wrists, and elbows can also be affected.

Recognising the symptom pattern

An acute flare often brings rapidly increasing pain, warmth, redness, swelling, and extreme tenderness in one or more joints. Even light contact from clothing or bedding may be uncomfortable. As gout becomes chronic, the joint may remain achy, stiff, enlarged, or harder to move after the intense inflammation eases.

Tophi can feel like firm, sometimes irregular lumps near fingers, toes, elbows, or the outer ear. They may cause no pain, but their position can interfere with footwear, grip, or joint motion. A tophus that breaks through the skin needs medical attention because the area can become irritated or infected.

What to do when symptoms change

Follow your prescribed flare plan early, rest the affected joint, and avoid pressure on it. Do not start, stop, or combine anti-inflammatory medicines without checking that they are suitable for your kidneys, stomach, heart, and other treatment.

Note when symptoms began, which joints are involved, whether fever or a wound is present, and whether discomfort fully resolves. Photos of visible swelling and a simple symptom diary can help a clinician distinguish separate attacks from continuing inflammation and judge whether long-term control needs adjustment.

Red flags and timely review

Get urgent medical assessment for a hot swollen joint accompanied by fever, chills, feeling seriously unwell, drainage, broken skin, or sudden inability to use the limb. Infection and gout may look similar, and infection must be excluded promptly.

Book a routine review for increasingly frequent attacks, persistent discomfort between flares, new lumps, loss of movement, or symptoms that do not respond as expected to the agreed plan. New ankle or knee swelling also deserves assessment when the cause is uncertain.

Can chronic gout involve more than one joint?

Yes. Established gout can affect several joints, either together or at different times, rather than remaining confined to one toe.

Are all gout lumps painful?

No. Tophi may not hurt, although they can limit motion, press on nearby tissue, or irritate the overlying skin.

Does redness prove that a swollen joint is gout?
