What it may mean

Loud or interrupted snoring may accompany obstructive sleep apnoea, in which relaxed throat tissues reduce airflow and the brain briefly prompts a waking response. A person may gasp, choke, wake with a dry mouth, or feel unrefreshed in the morning. Nasal blockage from allergic rhinitis or chronic sinusitis can make mouth breathing and snoring worse. Body weight can also contribute to airway narrowing, but choking during sleep should not be explained by weight alone.

A bed partner's observations are useful because the sleeper may not remember pauses or gasps. Note whether episodes recur, whether breathing pauses are seen, and whether daytime sleepiness, morning headaches, or difficulty concentrating is present.

What to do next

Arrange a primary-care appointment if choking, gasping, or loud snoring happens repeatedly. Bring a list of medicines and describe sleep position, nasal symptoms, alcohol use near bedtime, and daytime alertness. A clinician may examine the nose and throat and decide whether a sleep assessment is appropriate.

Until reviewed, keep a regular sleep routine, avoid alcohol or sedating medicines unless prescribed, and try side sleeping if it is comfortable. Do not drive when unusually sleepy. If weight is a factor, discuss a safe weight-management plan rather than making abrupt changes.

Seek urgent help if breathing difficulty continues after waking, lips or skin look blue or grey, chest pain occurs, or the person is difficult to wake. Arrange prompt medical advice for frequent nighttime choking, witnessed breathing pauses, severe daytime sleepiness, or episodes occurring with fainting or new confusion.
