Chickenpox before spots: early signs

The first symptoms are not specific to chickenpox. Fever and feeling unwell can occur with many infections, so they do not confirm the diagnosis on their own. The rash often develops afterward, beginning as small red marks that can become fluid-filled blisters and then scabs. Chickenpox commonly appears in crops, meaning new spots can arrive while older ones are changing. Shingles is another blistering condition, but its pattern and symptoms differ and need clinical assessment.

Record when the fever, tiredness and first skin changes started, and take a clear photograph if a rash appears. Limit contact with pregnant people, newborn babies and people with weakened immunity, because chickenpox can spread before the rash is fully established. Encourage fluids and rest. Get prompt medical advice if chickenpox is suspected in a newborn, pregnant person, older adult or someone with reduced immunity. Seek urgent help for breathing difficulty, severe dehydration, confusion, unusual sleepiness, a severe headache with neck stiffness, or a rash near the eye. Rapidly spreading redness, warmth, swelling or pus around a blister also needs assessment.

Ask whether the early symptoms fit chickenpox, when the person became contagious, how to protect household members, and which fever or itch treatments are suitable. Ask whether testing or antiviral medicine is needed and what changes should prompt urgent care.
