What Should You Know About CHF Leg Swelling?

Heart failure can present with swelling around the ankles, feet or lower legs. A weaker heart may move blood less effectively, raising pressure in nearby veins and allowing fluid to move into surrounding tissue. Swelling may be more noticeable after standing or sitting and may leave a temporary indentation when pressed.

Leg swelling has several possible causes. Varicose veins, kidney disease, some medicines, prolonged inactivity and obesity can also contribute. Swelling in both legs may fit a fluid-related cause, but its pattern alone cannot identify the reason. A clinician may ask when it began, whether it changes during the day, and whether your weight, breathing or exercise tolerance has changed.

If you have diagnosed heart failure, follow the fluid, salt and medicine instructions in your care plan. Do not start, stop or alter a water tablet without speaking with your prescriber. When resting, raising your legs may ease dependent swelling; gentle movement and avoiding long periods in one position may also help if your clinician has said they are suitable.

Keep a simple record of swelling, breathing symptoms and any change in your usual activity. Note whether shoes or socks fit differently and whether one leg is more swollen than the other. Arrange a review if swelling is new, persistent, gradually increasing or returning after treatment. The clinician may check your heart and lungs, examine the legs and review medicines or other health conditions.

Seek emergency help for severe or rapidly worsening breathlessness, chest pain, fainting, confusion, or blue or grey lips. These symptoms can signal a serious problem and should not be managed by waiting for leg swelling to settle.

Get prompt medical advice if swelling worsens quickly, comes with breathlessness when lying down, wakes you at night, or is accompanied by a marked fall in stamina. A single leg that becomes suddenly swollen, painful, warm or red needs urgent assessment because a clot or another local problem may be involved.

You can also ask whether your current medicines, salt or fluid plan needs review, whether leg elevation or compression is appropriate for you, and when your next heart-failure check should take place.
