How position can matter

Lying flat can make Acid Reflux more noticeable by allowing stomach contents to move upward, often with burning or a sour taste. Some people with asthma or COPD feel more breathless when lying down, particularly with cough or wheeze. Chest-wall strain may become obvious when the back or ribs are positioned a certain way. Angina is a heart condition that can present with pressure or tightness, and breathlessness while lying flat can have heart or lung causes. Panic Disorder and Generalised Anxiety Disorder may intensify awareness of breathing, but they should not be used to explain a new symptom without assessment.

What you can do

Move slowly to a comfortable upright position and stop activity if the sensation is present. Note whether it occurs only flat, during sleep, after meals, with exertion, or alongside cough and breathlessness. Avoid deliberately lying flat to reproduce it. Follow prescribed treatment instructions, including any Inhaler Therapy, and do not take another person's medicine. Seek a clinical review for repeated, new, or unexplained episodes.

Get urgent help when needed

Call emergency services if tightness is sudden, severe, persistent, or worsening, or if it comes with marked breathlessness, fainting, confusion, cold sweating, nausea, blue or grey lips, or discomfort spreading to the arm, jaw, back, or neck. Seek prompt assessment for coughing blood, fever with breathing difficulty, or new inability to lie flat because of breathing. Positional improvement does not cancel these warning signs.
