Chest tightness and trouble breathing

Short answer
Chest tightness with trouble breathing should be treated as urgent until the severity and cause have been assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First step
- Stop activity and sit upright
- Watch for
- Speech difficulty, blue lips, fainting, or spreading pressure
- Useful record
- Start time, trigger, and associated symptoms
Possible explanations
Breathlessness may accompany asthma, COPD, angina, reflux, chest-wall irritation, or anxiety. Asthma presents with variable tightness, wheeze, and cough, often after an airway trigger. COPD can cause breathlessness with activity and a persistent cough. Angina presents with pressure or tightness during exertion and may travel beyond the chest. Acid reflux can irritate the food pipe and create burning or pressure that is worse after meals or lying down. Panic disorder may bring sudden chest tightness, fast breathing, trembling, and a strong sense of fear. These patterns overlap, so the sensation alone cannot identify the cause.
What to do while waiting for help
Stop walking or exercising, sit upright, loosen tight clothing, and try to stay still. Follow your prescribed breathing action plan if you have one. Do not lie flat if that worsens breathing, and do not use someone else's medicine. Note whether you can speak in full sentences, when symptoms started, and whether wheeze, fever, cough, sweating, dizziness, or spreading pain is present.
Get emergency help
Call emergency services immediately for severe breathing difficulty, inability to speak normally, blue or grey lips, fainting, confusion, sudden weakness, or chest pressure spreading to the arm, jaw, back, or shoulder. Urgent assessment is also needed when symptoms are new, rapidly worsening, or start with exertion. Do not drive if you are struggling to breathe or feel faint.
Questions for your doctor
Ask whether your symptoms fit an airway, heart, or other cause; how to follow your inhaler plan; and what change means you should seek emergency care. Mention smoking or vaping, allergies, recent infection, and previous attacks.
