Chest tightness radiating to the back

Pain or pressure may come from the chest wall, the food pipe, the airways, or the heart. Acid reflux can create burning or pressure behind the breastbone, while muscle strain may worsen when you twist, lift, or press the area. Angina is chest discomfort caused by reduced blood flow to the heart and may spread toward the back. Asthma and COPD can produce chest tightness with wheezing or breathlessness. The direction of the discomfort does not identify the cause by itself.

Stop strenuous activity and sit upright. Note when the tightness began, whether it moves, what you were doing, and whether breathing, food, movement, or anxiety changes it. Arrange a medical assessment promptly if it is new, recurring, or unexplained. Do not drive yourself if symptoms are severe. Use a prescribed reliever only according to your action plan; an inhaler is not a test for a heart problem.

Call emergency services now for severe or persistent tightness, difficulty breathing, faintness, sweating, nausea, blue or grey lips, confusion, or discomfort spreading to the arm, jaw, or back. Seek urgent help as well if symptoms begin after an injury or with sudden weakness. These warning signs require assessment rather than watching at home.

Ask whether your symptoms fit reflux, a chest-wall problem, angina, asthma, or another cause; which examination or tests are appropriate; and what symptoms should trigger emergency help. Mention medicines, smoking or vaping, recent illness, exercise, meals, and any previous heart or lung condition.
