Chest tightness with difficulty breathing can be urgent, especially when breathing is severely limited or symptoms start suddenly. Sit upright, stop exertion and assess whether you can speak normally. Pay attention to wheeze, cough, faintness, sweating, confusion, colour change or discomfort that spreads beyond the chest.

What may be happening

Asthma can present with chest tightness, wheezing and difficulty moving air; COPD may cause a similar pattern, particularly with cough or phlegm. Angina can cause pressure with breathlessness during activity or sometimes at rest. Acid reflux may irritate the food pipe and create central pressure, while panic disorder or generalised anxiety disorder can cause fast breathing, tingling and a racing heartbeat. A strained chest muscle may hurt or tighten with movement, but breathlessness should not be attributed to muscle strain without assessment.

What to do

Sit upright, loosen tight clothing and try to stay still. Follow your prescribed asthma or COPD action plan if you have one, including your prescribed inhaler instructions. Do not use another person’s inhaler or delay help while trying home remedies. If the episode settles but is new, unexplained or recurrent, arrange prompt medical assessment and record what triggered it, how long it lasted and whether wheeze or cough occurred.

Emergency warning signs

Call emergency services immediately if you cannot speak in full sentences, breathing is rapidly worsening, lips or skin look blue or grey, you feel faint or confused, or the chest tightness is severe or persistent. Emergency help is also needed for cold sweating, sudden weakness, or discomfort spreading to the arm, jaw, shoulder or back. Do not drive yourself when breathing is seriously impaired.
