Possible explanations
Lying down can make acid reflux more noticeable and may bring pressure or burning behind the breastbone. Panic disorder or generalised anxiety disorder may cause sudden tightness with a racing heart, trembling or fear. Angina can present with chest pressure and may occur during activity or at rest. The pattern helps a clinician, but it does not establish a diagnosis.
Steps to take
Keep a short record of bedtime, meals, sleep position, symptoms and duration. Avoid lying down immediately after a large meal, and sit upright while symptoms are present. Follow any written asthma or COPD plan and use prescribed inhalers as directed; inhaler therapy technique can affect how much medicine reaches the lungs. Arrange a clinical review if episodes repeat, disturb sleep or are becoming more intense.

Get emergency help for chest tightness with severe breathlessness, fainting, confusion, blue or grey lips, cold sweating, sudden weakness, or discomfort spreading to the arm, jaw, shoulder or back. Seek urgent assessment if symptoms are new, prolonged, rapidly worsening or present with a known heart or lung condition. Sit upright and avoid driving if you feel unsafe.
