What wheezing may indicate

Wheezing is a whistling sound produced when air moves through narrowed airways. Asthma may cause episodes after exercise, cold air, allergens, or respiratory infections, while COPD can cause ongoing airflow limitation, cough, and mucus. Acid reflux may irritate the throat and airways in some people. Anxiety can change breathing and increase the sense of chest constriction, but it does not explain every wheeze. The sound may be louder when breathing out, and a clinician may need to listen to your lungs and test airflow.

What to do

Stop activity, sit upright, and move away from smoke, dust, or a known trigger. Use a prescribed reliever inhaler only according to your action plan; do not borrow one or take extra doses without advice. Note the trigger, cough, mucus, fever, chest pain, and response to prescribed treatment. Arrange an appointment for a first episode, repeated wheezing, nighttime symptoms, or symptoms that limit walking or sleep.

When to seek urgent care

Call emergency services if breathing becomes severe, you cannot speak in normal sentences, your lips look blue or grey, you become faint or confused, or wheezing begins with swelling of the face, mouth, or throat. Seek urgent help when chest tightness is severe, rapidly worsening, or not improving with your prescribed action plan. A quiet chest during extreme breathlessness can also be dangerous because very little air may be moving.
