Chest tightness and fatigue: what to know

Short answer
Chest tightness with fatigue has several possible explanations, and new severe symptoms or breathlessness should be assessed urgently.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the combination can suggest
Fatigue may follow poor sleep, stress, dehydration or a recent infection. When it appears with chest tightness, clinicians also consider breathing conditions such as Asthma or COPD, heart-related symptoms such as Angina, and less urgent causes such as reflux or muscle tension. Generalised Anxiety Disorder and Panic Disorder can produce exhaustion after episodes of intense physical arousal, but that explanation should fit the complete history.
Think about whether the tightness occurs with walking or stairs, whether rest helps, and whether fatigue is new or steadily worsening. Record sleep, fever, cough, wheeze, palpitations, dizziness and changes in daily activity.
Next steps
Pause strenuous activity, drink fluids if you may be dehydrated, and keep a record of symptoms and triggers. Book a medical review for unexplained or recurring symptoms, especially when ordinary activities become harder. A clinician may check your heart, lungs, medication effects, sleep and other causes of low energy.
Urgent warning signs
Call emergency services for severe chest pressure, fainting, confusion, severe breathlessness, cold sweating, or discomfort spreading to the arm, jaw, back or neck. Seek prompt care when fatigue is sudden and profound, or when chest tightness accompanies a worsening cough, fever, palpitations or reduced ability to walk normally.
Questions for your appointment
Could the timing with activity distinguish a heart or lung problem from poor sleep or stress? Which symptoms should I monitor at home? Do my medicines, sleep pattern or recent illness need review?

Questions people ask
Can tiredness occur after a panic episode?
The physical strain of a panic episode can leave someone tired, but ongoing or new chest tightness should not be attributed to panic without assessment.
Does fatigue with chest tightness need a check-up?
Arrange a review when the symptoms are unexplained, recurring, worsening or limiting normal activity; urgent help is needed for severe associated symptoms.
