Chest tightness after a pacemaker: what to know

A pacemaker helps control an abnormal heart rhythm, but it does not prevent every cause of chest discomfort. Tightness can occur with angina, acid reflux, airway narrowing, anxiety, or irritation around the procedure area. A pacemaker problem is less common, but new symptoms still deserve review by the implanting team. Note whether the feeling began suddenly, follows exertion, changes with breathing or position, or comes with palpitations, dizziness, fever, or swelling.

Stop exertion, sit upright, and follow any discharge instructions from your cardiology team. Contact the pacemaker clinic promptly for new, recurring, or unexplained tightness; have your device card and medication list available. Do not press or manipulate the implant site. Record the timing, duration, activity, and associated symptoms so the clinician can decide whether an examination, heart tracing, chest imaging, or device check is needed.

Call emergency services immediately for severe or worsening tightness, trouble breathing, fainting, marked weakness, confusion, blue or grey lips, or discomfort spreading to the arm, jaw, back, or shoulder. Seek urgent help as well if the wound is opening, rapidly swelling, hot, or draining, or if you have a high fever. Do not drive yourself while seriously unwell.

Ask whether your symptoms could be related to the rhythm, the device leads, the wound, or a separate condition.
