How to interpret it
Acid Reflux can cause a burning or tight sensation behind the breastbone and upper stomach, often after meals or when lying down. Asthma or COPD may cause chest tightness with wheeze, cough or breathlessness. Generalised Anxiety Disorder and Panic Disorder can produce chest and abdominal muscle tension, fast breathing, trembling or a sudden wave of fear. Angina may feel like pressure or squeezing, particularly with exertion. The same symptom can have different causes, so its pattern cannot diagnose the cause by itself.
What to do now
Sit upright, loosen tight clothing and note whether meals, exertion, stress, position or breathing trigger the sensation. Avoid strenuous activity while unexplained chest tightness is active. If you have a prescribed reliever plan, use it exactly as directed; a clinician may review whether Inhaler Therapy is suitable. Arrange medical review for repeated episodes, symptoms during activity, swallowing difficulty, persistent vomiting or symptoms that disturb sleep.
When to seek care
Seek emergency help for chest tightness that is severe, new, persistent or worsening, especially with breathlessness, faintness, sweating, nausea, blue lips, or pain spreading to the arm, back, neck or jaw. Urgent assessment is also needed for coughing blood, sudden severe abdominal pain, a rigid abdomen or inability to keep fluids down. Do not assume anxiety or reflux is the cause before urgent features have been considered.
