Cervical bleeding after sex: causes and next steps

Postcoital bleeding may come from delicate or inflamed tissue on the cervix, vaginal dryness or injury, a cervical or uterine polyp, an infection such as chlamydia, or hormonal changes. Genitourinary syndrome of menopause can make vaginal tissue thinner, drier and easier to injure. Bleeding can also occur with cervical dysplasia, cervical cancer or vaginal cancer, so repeated bleeding deserves assessment even when it is light.

The symptom alone cannot identify the cause. A clinician may ask about menstrual timing, contraception, pregnancy possibility, discharge, pain, menopause and cervical screening. Depending on the history, assessment can include a speculum examination, pregnancy test, infection swabs and checking whether cervical screening is due. An abnormal-looking area may require specialist examination or a tissue sample.

Note when the bleeding occurs, its colour and amount, whether it happens every time, and whether there is pain, unusual discharge or bleeding between periods. Record the date of your last period, contraception, pregnancy possibility and latest cervical screening result if known. Avoid assuming the cervix is the source; blood may arise from another part of the genital tract.

Book a clinical review for recurrent, ongoing or unexplained bleeding, including any bleeding after sex once periods have stopped with menopause. Until assessed, stop sexual activity if it causes pain or further bleeding. If dryness seems relevant, a water-based lubricant may reduce friction, but it should not delay evaluation of repeated bleeding.

Seek urgent care for heavy bleeding, severe pelvic or abdominal pain, fainting, marked dizziness, fever or feeling acutely unwell. If pregnancy is possible, bleeding with one-sided pelvic pain, shoulder-tip pain, weakness or collapse needs emergency assessment because an ectopic pregnancy must be excluded. New bleeding after menopause should be arranged for prompt medical assessment even if it happens only once or stops.

Ask whether the examination identified the source of bleeding, whether pregnancy and infection tests are appropriate, and whether cervical screening is current. Clarify if referral for colposcopy, ultrasound or biopsy is needed, how results will be communicated, and what change in bleeding should trigger urgent care. If dryness or a benign cervical change is found, request treatment options matched to that cause.
