What should you know about central lung cancer?

The word central identifies the tumour's location rather than a separate cell type. A centrally placed tumour may narrow a bronchus, irritate nearby nerves or vessels, or block part of the lung from ventilating normally.

How location shapes diagnosis

Central lung cancers can present with a persistent or changing cough, coughing up blood, breathlessness, repeated infection behind a blocked airway, chest discomfort or a hoarse voice. These symptoms also have non-cancerous causes, so imaging and tissue sampling are needed to establish a diagnosis.

A chest scan shows the tumour's relationship to airways, blood vessels and lymph nodes. Bronchoscopy may let the clinical team see an airway abnormality and collect tissue. The pathology result identifies the cancer type, while staging tests show whether it is confined to the chest or has spread.

What to do after a suspected or confirmed finding

Attend the respiratory or oncology assessment and bring prior scans, pathology reports and a current medicines list. Tell the team about breathing changes, blood in sputum, weight or appetite changes, pain and how symptoms affect daily activity.

Treatment is matched to the cell type, stage, molecular features, exact relationship to central structures and overall fitness. Options can include surgery when the tumour can be removed safely, radiotherapy, drug treatment such as chemotherapy or targeted treatment, and supportive care for breathlessness, cough, pain or fatigue.

When to seek urgent care

Seek emergency help for severe or rapidly worsening breathlessness, coughing up more than small streaks of blood, crushing chest pain, fainting, blue or grey lips, new confusion, or swelling of the face or neck with breathing difficulty. These signs can reflect airway obstruction, significant bleeding, a blood clot or pressure on a major chest vessel.

Contact the treating team promptly for fever with a worsening cough, new hoarseness, increasing wheeze, repeated blood-streaked sputum or a clear decline in usual activity. Do not wait for a scheduled appointment when symptoms are progressing.
