Can malignant pleural effusion be prevented?

The aim is to control the underlying disease when possible, make breathing easier and choose a fluid-management plan suited to the person's symptoms, general health and how quickly the fluid reaccumulates.

The pleura are thin layers surrounding the lungs. Cancer can inflame these layers or block normal fluid drainage, so fluid gathers in the pleural space and restricts lung expansion. Lung cancer, breast cancer and mesothelioma can present with a malignant pleural effusion.

Breathlessness, cough and chest heaviness can develop, but symptom intensity does not always match the volume of fluid. Infection, a blood clot and other lung problems can produce similar symptoms and need different treatment.

Report new or worsening breathing difficulty to the oncology team. Imaging can show whether fluid is present, and draining a sample can help establish the cause while also relieving pressure when the collection is large enough to cause symptoms.

If fluid returns, options may include another drainage, an indwelling pleural catheter or pleurodesis, a procedure that helps the pleural layers adhere so fluid has less space to collect. The choice depends partly on whether the lung expands after drainage and how much home management is practical.

Call the treating team soon for gradually worsening breathlessness, a persistent new cough, chest pressure, reduced ability to manage usual activity, or concerns about a pleural catheter. A fever, cloudy drainage, redness or increasing pain around a catheter site needs prompt advice.

Call emergency services for sudden or severe breathlessness, blue or grey lips, fainting, confusion, coughing blood, or abrupt chest pain. Sit upright if that eases breathing while help is arranged.

Ask whether the lung expanded fully after drainage, how likely the fluid is to return in your situation, and how each procedure could affect daily life. Clarify catheter care, signs of infection, who supplies drainage equipment and whom to contact if breathing worsens outside clinic hours.
