The short answer
It may be performed urgently when pressure rises suddenly after an injury, or as a planned procedure for exertional symptoms. The urgency, incision plan, and recovery pathway depend on why pressure developed and what the surgeon finds.
How the operation works
Muscles in the calf sit inside firm sleeves called fascia. Swelling inside this limited space can compress small blood vessels and nerves. During fasciotomy, the surgeon opens the fascia through one or more skin incisions so the affected compartments have room to expand.
With severe acute swelling, the wound may remain open temporarily and be checked again before closure or skin coverage. A planned procedure for exercise-related pressure is commonly closed at the operation. These are distinct situations, so another patient's recovery may not match yours.
Recovery and wound care
Follow the surgical team's instructions on dressings, washing, elevation, medicines, and how much weight to place through the leg. Keep the dressing clean and dry unless you receive different instructions. Do not apply creams or massage near an unhealed incision.
Movement is usually advanced in stages. Physiotherapy may address ankle motion, swelling, walking, strength, and a gradual return to work or sport. Attend wound checks because skin healing, sensation, muscle function, and circulation all influence the next step.
When to seek urgent care
Before surgery, rapidly worsening calf pain, a tense swollen calf, pain out of proportion to the injury, numbness, weakness, or difficulty moving the foot needs emergency assessment. Do not wait for a routine appointment or try to release the pressure yourself.
After surgery, contact the surgical team promptly for spreading redness, wound discharge, fever, increasing swelling, or pain that is escalating rather than settling. Seek emergency help for a cold or pale foot, new loss of feeling or movement, sudden breathlessness, chest pain, or heavy bleeding through the dressing.
Questions for your surgical team
Ask which calf compartments were released, whether the wound will need another procedure, and what changes in foot colour, warmth, sensation, or movement should trigger a call. Confirm when you may shower, drive, work, bear weight, and restart exercise.
It is also useful to clarify who will change the dressing, when stitches or clips will be reviewed, and whether you need crutches, a splint, or physiotherapy.
