Calcific tendonitis and pain down the arm

How the structures involved in Tendinopathy differ from normal

Short answer

Calcific tendonitis can cause intense shoulder pain that spreads down the upper arm, but pain reaching the hand warrants assessment for another source.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why the pain can travel

Calcific tendonitis occurs when calcium deposits form within a tendon, often in the shoulder's rotator cuff. Irritation around the tendon and nearby bursa may produce pain at the shoulder that is felt over the outer upper arm. Reaching overhead, lifting, lying on that side, or moving the arm away from the body may aggravate it.

Pain caused by the shoulder commonly stops above the elbow. Tingling, numbness, altered hand sensation, or pain extending from the neck into the fingers can point to nerve irritation or a separate neck problem. An examination helps distinguish these patterns; imaging may be used to locate a calcium deposit and check other structures.

What to do during a flare

Temporarily reduce painful overhead work and heavy lifting, while keeping the arm moving gently within a comfortable range. A wrapped cold pack can ease a hot, irritated shoulder. Discuss pain medicine with a pharmacist or clinician if you have stomach, kidney, heart, bleeding, pregnancy, or medication concerns.

A physiotherapist can assess shoulder and neck movement, then progress mobility and loading as symptoms settle. Depending on the examination and imaging, a clinician may discuss guided needling, shockwave therapy, an injection, or surgical referral when simpler measures have not provided enough relief.

When to seek medical care

Arrange an assessment if pain is severe, repeatedly disturbs sleep, limits daily use of the arm, follows an injury, or is not settling. Seek urgent care for a suddenly weak or numb arm, a visibly deformed shoulder, a cold or pale hand, fever with a red or hot swollen joint, chest pressure, breathlessness, sweating, or pain spreading from the chest to the arm or jaw.

Questions for your appointment

Ask whether the pain pattern fits the shoulder alone, whether the neck and nerves need checking, and whether an X-ray or ultrasound would change management. It is also useful to clarify which movements to maintain, which loads to pause, and what improvement should prompt progression of rehabilitation.

Questions people ask

Can calcific tendonitis pain go below the elbow?

It can be difficult to locate pain precisely, but symptoms below the elbow, especially tingling or numbness, should prompt assessment for nerve or neck involvement.

Should I stop moving my shoulder completely?

Complete rest can increase stiffness. Reduce clearly painful loading, but continue comfortable movement unless a clinician has advised immobilisation after an injury.

Does an X-ray confirm that the deposit causes the pain?

An X-ray can show a calcium deposit, but the finding must be matched with the examination because a visible deposit may not explain every symptom.
