Calcific longus colli tendinitis: symptoms, care and warning signs

Short answer
Calcific longus colli tendinitis is temporary inflammation around a calcium deposit in a deep neck tendon, often causing sudden neck pain, stiffness and painful swallowing.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- A deep tendon at the front of the upper cervical spine
- Typical features
- Sudden neck pain, stiffness and pain with swallowing
- Usual approach
- Clinical assessment, imaging when needed and non-surgical symptom control
The short answer
It can resemble a throat infection or another serious neck disorder, so a clinician may use an examination and imaging to confirm the cause. Treatment is generally non-surgical and aims to settle inflammation while keeping symptoms manageable.
What is happening in the neck
The longus colli muscles lie in front of the cervical spine and help bend and stabilise the neck. Calcium crystals can collect in a tendon near the upper neck and trigger a local inflammatory reaction. The resulting swelling may make turning the head difficult and swallowing uncomfortable.
Neck pain, restricted movement and discomfort on swallowing are recognised presentations of this condition. Those features also occur in infections and other neck problems, however, so symptoms alone cannot establish the diagnosis.
Practical next steps
Arrange a prompt clinical assessment for new severe neck stiffness or painful swallowing. The clinician may check temperature, breathing, neurological function and the throat, then decide whether blood tests or neck imaging are appropriate. Computed tomography can show the characteristic deposit and nearby swelling.
Follow the prescribed plan, which may include an anti-inflammatory medicine if it is safe for you, short-term activity modification and simple pain relief. Do not force neck stretches during the painful phase. Resume comfortable movement gradually as pain eases, and check before taking anti-inflammatory medicines if you have kidney disease, stomach ulcers, take blood thinners or are pregnant.
When care cannot wait
Seek emergency care for breathing difficulty, inability to swallow saliva, drooling, rapidly increasing neck swelling, confusion, fainting, new limb weakness or numbness, loss of coordination, or a severe headache with marked neck stiffness. Fever, chills or feeling acutely unwell also need urgent assessment because infection must be excluded.
Contact your clinician again if pain is worsening despite treatment, swallowing is becoming harder, or symptoms fail to follow the expected recovery pattern.
Useful questions for your appointment
Ask what examination or imaging findings support the diagnosis, which serious alternatives have been considered, and which medicine is suitable alongside your current prescriptions. You can also ask when to restart driving, work and exercise, and which change in symptoms should trigger reassessment.

Questions people ask
Is calcific longus colli tendinitis an infection?
No. It is an inflammatory reaction around a calcium deposit, but its symptoms can overlap with a deep neck infection. A clinician should distinguish between them.
Will I need surgery?
Surgery is generally not used when the diagnosis is confirmed. Management commonly focuses on reducing inflammation and pain while the episode settles.
Should I stretch a very stiff neck?
Avoid forceful stretching while pain is acute. Gentle movement within a comfortable range may be introduced according to your clinician's advice.
