C5 Cervical Radiculopathy: Symptoms, Assessment and Care

Short answer
C5 cervical radiculopathy is irritation or compression of the C5 nerve root, which can cause neck-to-shoulder pain and weakness when lifting the arm.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Area involved
- A nerve root in the lower part of the neck
- Possible pattern
- Pain near the shoulder with difficulty lifting the arm
- Assessment
- Strength, sensation, reflexes and shoulder examination
The short answer
The pattern varies between people and overlaps with shoulder disorders, so an examination matters more than any single symptom. Many cases are first managed without surgery, depending on the cause, severity and whether weakness is progressing.
How the C5 nerve root can affect you
The C5 nerve root leaves the neck near the C4–C5 level. When affected, cervical radiculopathy may present with pain around the lower neck, top of the shoulder or outer upper arm. Numbness is less prominent in some people than pain or weakness.
Weakness may involve the deltoid muscle used to raise the arm and sometimes the biceps used to bend the elbow. A clinician may compare strength, sensation and reflexes on both sides, then examine the shoulder because rotator cuff problems can create a similar functional difficulty.
Practical next steps
Arrange a clinical assessment if pain persists, travels from the neck into the shoulder, or is accompanied by weakness or altered sensation. Note which movements provoke symptoms and whether you are dropping objects or struggling to lift the arm.
Until assessed, avoid forceful neck manipulation and activities that clearly worsen arm symptoms. Comfortable movement, posture changes and prescribed physiotherapy may help maintain function. Medicines, imaging or specialist review are selected according to the examination rather than used automatically.
When to seek urgent care
Seek urgent medical assessment for new or rapidly worsening arm weakness, difficulty walking, loss of balance, clumsy hands, or changes in bladder or bowel control. These features can indicate pressure affecting more than one nerve root or the spinal cord.
Emergency care is also appropriate after significant neck trauma, or when neck pain occurs with fever, severe illness, or a sudden neurological change. Urgency depends on these warning features, not on whether C5 radiculopathy is a common explanation.
Questions to take to your appointment
Ask which examination findings point toward C5 rather than a shoulder problem, whether your strength is changing, and what activities are safe. You can also ask what would make imaging useful, what improvement to monitor, and which warning signs should bring you back sooner.

Questions people ask
Can C5 radiculopathy feel like a shoulder injury?
Yes. Both can cause shoulder-region pain and difficulty raising the arm. Examining neck movement, shoulder structures, strength, sensation and reflexes helps distinguish the patterns.
Does C5 cervical radiculopathy always require surgery?
No. Treatment is guided by the cause and neurological findings. Non-surgical care may be appropriate when symptoms are stable, while progressive weakness or spinal cord signs need prompt specialist assessment.
