C4 complement blood test: what should you know?

Short answer

A C4 complement blood test measures one protein in the immune complement system and must be interpreted with your symptoms and other tests.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What C4 can add

C4 is one of the proteins that participates in complement activity. A clinician may check it when investigating inflammation, immune-system problems, repeated infections, or findings involving the kidneys. A lower value can occur when complement is being used in an active process; a higher value can accompany inflammation. These patterns are not specific, and a C4 result does not name a disease. C3 is often considered alongside it, with antibody tests, blood counts, urine tests, and examination findings providing additional context. You can read about [Chronic Kidney Disease](/conditions/chronic-kidney-disease/) or [Brucellosis](/conditions/brucellosis/) as examples of conditions that may be assessed with broader testing.

Getting ready

C4 is measured from a blood sample taken from a vein. Ask whether you should fast, particularly if several tests are planned. Mention medicines, supplements, recent fever, skin changes, joint symptoms, and any previous immune or kidney findings. When the report arrives, ask the ordering clinician to explain the result using that laboratory's range and your clinical picture. Do not alter prescribed treatment based on C4 alone.

When to seek care

Seek medical advice for ongoing fever, unexplained swelling, a new rash, persistent joint pain, or urine that becomes unusually dark or reduced. Emergency assessment is needed for breathing difficulty, swelling around the mouth or throat, collapse, severe drowsiness, or a rapidly worsening condition. The urgency comes from these symptoms, not from an isolated laboratory value.

Questions worth asking

Why was C4 selected for my assessment? Was C3 measured at the same time? Is the value outside the local reference range? Could illness or treatment have influenced it? Should I have repeat blood work, urine testing, or specialist review?

Questions people ask

Is a low C4 result a diagnosis?

No. It is a clinical clue that needs to be considered with C3, symptoms, medical history, and other investigations.

Can I eat before a C4 blood test?

Often the test itself does not require fasting, but follow the instructions for the full group of tests your clinician ordered.
