VBAC after a C-section: assessment and birth planning

VBAC describes the vaginal birth itself. The planned attempt is often called a trial of labour after caesarean. It can avoid another abdominal operation when vaginal birth occurs, while a repeat caesarean offers a scheduled surgical route. Either plan has potential benefits and harms that depend on personal circumstances.

A central concern during labour is separation of the old uterine scar. This is uncommon but can be serious, which is why the uterine incision type, previous surgical history and ability to perform an urgent caesarean are considered. The appearance of the abdominal skin scar does not show how the uterus was opened.

Review the earlier caesarean report with an obstetric clinician and discuss why it was needed. Assessment also considers previous vaginal births or uterine surgery, placenta position, fetal presentation, other pregnancy conditions and your priorities.

Choose a maternity setting equipped for appropriate fetal monitoring and urgent surgery. Agree what happens if labour does not start as expected, which induction or augmentation methods may be considered, what pain relief is available and when the team would recommend moving to caesarean birth. Record preferences while allowing for changes if clinical circumstances shift.

Before labour, seek urgent maternity advice for vaginal bleeding, severe or persistent abdominal pain, fluid leakage, reduced fetal movement, faintness or feeling seriously unwell. Use the contact instructions provided by your maternity unit rather than waiting for a routine visit.

Once labour starts, report pain that feels continuous or different from contractions, sudden intense scar-area pain, bleeding, dizziness or breathing difficulty immediately. Some concerns are first detected through fetal heart-rate changes, so recommended monitoring is part of the safety plan.

Ask whether your uterine incision and operation history support a trial of labour, which factors could change that advice, and how your hospital manages VBAC. Request a clear explanation of monitoring, induction, pain relief, urgent surgical access and recovery differences between vaginal birth and repeat caesarean.
