Swelling after a C-section: what to expect and when to get help

What the swelling may mean
Pregnancy causes the body to retain fluid, and fluid given through a drip around surgery can add to it. After birth, gravity may draw this fluid into the feet, ankles and lower legs, especially after lying or sitting for long periods. Puffiness on both sides that gradually eases is often part of postpartum fluid adjustment.
Swelling deserves closer attention when one leg is more affected, the calf is painful or tender, the skin feels unusually warm, or the swelling is rapidly worsening. These changes can occur with a clot in a deep leg vein. Swelling of the face or hands alongside a persistent headache, visual disturbance, upper abdominal pain, nausea or breathlessness can accompany postpartum pre-eclampsia, including after an earlier normal blood-pressure history.
Comfort and recovery measures
Take short, gentle walks as your maternity team has advised, change position regularly, and move your ankles while resting. Raise your lower legs on pillows without putting pressure on the surgical wound. Drink according to thirst and follow any personalised advice about fluids, blood-pressure medicines, compression stockings or clot-prevention injections.
Do not massage a leg that is newly painful, hot or swollen. Keep checking whether swelling is equal on both sides and whether it is improving. Contact your maternity team the same day if puffiness is increasing, interferes with walking, occurs with reduced urination, or you are uncertain whether it matches the recovery plan given at discharge.
Red flags after a C-section
Seek emergency help for chest pain, sudden shortness of breath, coughing blood, collapse, a seizure, or severe difficulty breathing. These symptoms may signal a clot in the lungs or another serious postpartum complication.
Arrange urgent assessment for swelling, pain, redness or warmth mainly in one leg; a severe or persistent headache; blurred vision or flashing lights; pain high in the abdomen; sudden face or hand swelling; or a blood-pressure reading above the limit your maternity team gave you. Also report fever, worsening wound redness or swelling, pus, bad-smelling discharge, heavy vaginal bleeding, or escalating abdominal pain promptly.

Ask whether the pattern of swelling fits expected fluid loss after your operation, whether your blood pressure should be checked at home, and which reading should trigger a call. Confirm how much walking is suitable, whether you should continue stockings or injections, and whom to contact outside clinic hours if one leg changes or new headache or vision symptoms appear.

Why can my feet swell more after the C-section than before birth?
Fluid retained during pregnancy and fluid given around surgery can move into the lower legs before the kidneys gradually remove it. Reduced movement after the operation may make ankle and foot puffiness more noticeable.
Is swelling in only one leg normal after a caesarean birth?
