Drainage from a C-section incision

A clinician considers the amount, colour, thickness and smell of the fluid, along with the condition of the surrounding skin. A small spot of thin, clear or pale fluid can come from healing tissue, but only an examination can determine whether the wound is closing properly. Pus-like or unpleasant-smelling drainage can accompany infection, while continued blood or a suddenly soaked dressing needs urgent attention.

Other clues matter: increasing redness, warmth, swelling or pain around the cut; edges that are pulling apart; fever; chills; or feeling generally unwell. Fluid from the vagina is separate from fluid emerging through the abdominal incision, so identify where the discharge originates when speaking to the maternity team.

Wash your hands before and after touching the dressing area. Follow the wound-care instructions provided at discharge, keep the area gently clean and dry, and use a clean dressing if your team has advised one. Do not squeeze the wound, probe an opening, pull at stitches, or apply powders, oils, antiseptics or creams unless prescribed.

Contact maternity triage, your midwife or the surgical team and describe when drainage started, its appearance and whether the dressing becomes wet again. Mention pain, odour, fever, skin changes and any separation of the wound edges. A clinician may inspect the incision and decide whether a swab, dressing plan, antibiotics or drainage of a collection is appropriate.

Seek emergency care if bleeding is heavy or rapidly saturates the dressing, the wound opens widely, you faint, become confused, have severe worsening pain, or feel acutely unwell. Obtain prompt same-day assessment for pus, foul odour, fever, spreading redness, increasing heat or swelling, worsening tenderness, persistent leakage, or even a small opening in the incision. Do not wait for a routine postnatal appointment when these changes are present.

Ask whether the wound edges and deeper layers remain closed, whether the fluid suggests infection or a collection, and how often the dressing should be changed. Confirm how to bathe, which activities may strain the incision, when follow-up will occur and exactly which changes should trigger another urgent call.
