What should you know about BV and trichomoniasis treatment?

Treatment should follow a confirmed or clinically supported diagnosis because BV reflects a bacterial imbalance and trichomoniasis is caused by a sexually transmitted parasite. If testing finds both, the prescriber will choose a plan that covers both conditions and fits pregnancy status, allergies and other medicines.

BV may be treated with an oral antibiotic or a vaginal antibiotic preparation. Trichomoniasis requires an effective oral medicine because vaginal gel does not adequately treat infection in all affected genital areas. The prescriber selects the medicine and course after considering your history and local guidance.

Routine treatment of a male partner is generally not part of BV care. With trichomoniasis, current sexual partners require treatment even when they feel well, and sex should wait until everyone has completed therapy and symptoms have resolved. This distinction helps prevent trichomoniasis passing back and forth.

Take each dose exactly as labelled and complete the prescribed course. If you miss a dose, vomit after taking medicine or develop a troublesome side effect, use the leaflet instructions or contact the prescriber rather than doubling a dose. Tell the clinician if you are pregnant, breastfeeding, allergic to an antimicrobial or taking medicines that may interact.

Do not douche or use perfumed products inside the vagina. For trichomoniasis, make sure partners arrange treatment and follow the clinician's advice on repeat testing. If symptoms recur after BV therapy, return for review; repeated self-treatment can overlook trichomoniasis, thrush, cervicitis or a non-infectious cause.

Contact the treating service if symptoms fail to improve, return soon after the course, or appear after sex with an untreated partner. A new sample may be needed before another prescription, since persistent symptoms can reflect reinfection, a different vaginal condition or reduced response to the medicine.

Seek urgent help for facial or throat swelling, breathing difficulty, collapse or a widespread blistering rash after medication. Severe pelvic pain, high fever, heavy bleeding, persistent vomiting or marked weakness also warrants urgent assessment rather than waiting for the antibiotic course to end.
