Butt pain during ovulation: possible explanations

The ovaries, uterus, pelvic floor and rectum share nearby nerves and muscles. Irritation around the time an ovary releases an egg can therefore be perceived deeper in the pelvis, lower back, buttock or rectal area. A brief ache or sharp sensation that follows a similar mid-cycle pattern may fit ovulation pain.

Timing alone cannot confirm the cause. Endometriosis can present with cyclical pelvic or rectal pain, especially alongside painful periods, pain when opening the bowels or pain during sex. Ovarian cysts can present with one-sided pelvic pain or pressure. Constipation, an anal fissure, haemorrhoids and pelvic-floor spasm may also cause pain in the same region and may be unrelated to ovulation.

Record when the pain begins, where it is felt, how long it lasts and whether bowel movements, sex or movement change it. Also note bleeding, discharge, nausea, urinary symptoms and the dates of menstrual bleeding. This pattern can help a clinician separate a cycle-linked symptom from a bowel, muscular or ovarian problem.

Gentle heat and comfortable movement may ease a mild episode. A pharmacist can help you choose pain relief that is appropriate if you could be pregnant, have stomach or kidney problems, take other medicines or have allergies. Avoid anal penetration while the area hurts.

Get urgent medical care for sudden severe pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, persistent vomiting, a swollen abdomen, or pain with a positive pregnancy test or possible pregnancy. These features can accompany conditions that need rapid evaluation. Book a routine appointment if pain returns over several cycles, disrupts daily activity, occurs between cycles, or comes with painful bowel movements, sex or periods.

Consider asking whether the pattern suggests ovulation pain, pelvic-floor tension, endometriosis, an ovarian cyst or a bowel problem. Clarify whether a pregnancy test, pelvic examination or imaging would add useful information, and which pain relief fits your medical history.
