Butt bleeding after sex: causes and when to get help

First establish whether the blood is from the anus, nearby skin, vagina or urinary opening. Blood seen on tissue or bedding can spread, making its origin hard to identify. Anal penetration may cause surface irritation or an anal fissure, particularly when there was limited lubrication or pain. Haemorrhoids can also bleed after friction or pressure.

If the blood is vaginal rather than rectal, possible explanations include dryness, inflammation, infection or changes on the cervix. Chlamydia can present with bleeding after sex, while genitourinary syndrome of menopause often causes dryness and fragile tissue. Cervical dysplasia, cervical cancer and vaginal cancer can also present with bleeding after sex, so recurrent vaginal bleeding deserves assessment; the symptom alone does not establish any diagnosis.

Pause penetration until bleeding and soreness have settled. Wash gently with water, avoid inserting products into the anus or vagina, and note the colour, amount and suspected source of blood. For future anal sex, generous compatible lubricant, slower penetration and stopping when pain begins can reduce tissue injury. Do not use numbing cream because it can hide pain that signals damage.

Arrange a clinician visit if you cannot identify the source or if the bleeding returns. An examination may include the skin and anus, and, when vaginal bleeding is possible, the vagina and cervix. Testing may be offered when an infection is a possibility.

Seek urgent care for heavy or continuing bleeding, dizziness, faintness, severe abdominal or rectal pain, fever, black or tar-like stool, or an object that may remain inside. Get prompt assessment for a deep tear, pus, increasing swelling, loss of bowel control, or bleeding during pregnancy. Even without these signs, book an appointment when bleeding repeatedly follows sex or happens at other times.
