Outer-hip bursitis at a glance

The ache may extend down the outside of the thigh but should be assessed if its pattern is unclear. Nearby gluteal tendons are often considered because tendon pain and bursal irritation can occur in the same region and need a coordinated rehabilitation approach.

The tissues at the outer hip

A bursa is a low-friction cushion between tissues. At the outside of the hip, it lies near the greater trochanter, where tendons and other soft tissues pass over bone. Compression and repeated loading can make this area sensitive.

Pain from the hip joint itself is often felt differently, and lower-back or nerve problems can also send symptoms toward the hip. During assessment, a clinician checks tenderness, hip movement, muscle loading, walking control, back signs, and the effect of lying pressure rather than relying on one manoeuvre.

Calm the area without losing capacity

Reduce direct compression by avoiding the painful side in bed and not leaning the outer hip against counters or car doors. When lying on the other side, place a supportive pillow between the knees so the upper thigh does not drop across the body. Break up long periods of standing if they steadily increase the ache.

Keep movement within a tolerable range and build back gradually. Physiotherapy may use staged gluteal and trunk strengthening while adjusting positions that compress the side of the hip. Anti-inflammatory medication has health and interaction considerations, so check suitability first. A cortisone injection can be considered for selected patients after examination, alongside a plan for the contributing load.

Know when it may be something else

Arrange care if outer-hip pain is worsening, repeatedly wakes you, makes you limp, or fails to improve with sensible load changes. A fall followed by difficulty bearing weight needs urgent evaluation for injury rather than an assumption that a bursa has flared.

Seek prompt medical attention for fever with a hot or red hip area, rapidly developing swelling, severe pain at rest with feeling unwell, new weakness or numbness, or changes in bladder or bowel control. These findings require assessment for causes beyond uncomplicated outer-hip bursitis.

Plan an informed recovery

Ask whether the gluteal tendons are involved, which sleeping and standing positions are adding compression, and how much discomfort is acceptable during rehabilitation. It is also useful to clarify whether imaging would alter the plan and when an injection or specialist referral would be considered.

Can outer-hip bursitis cause pain down the leg?

The ache can spread along the outer thigh. Symptoms that travel below the knee with tingling, numbness, or weakness may suggest nerve involvement and deserve assessment.

Are stretches helpful for outer-hip bursitis?

Some stretches pull the thigh across the body and compress the already tender outer hip. A physiotherapist can select mobility and strengthening work based on which movements reproduce your symptoms.

Why does standing on one leg aggravate my hip?

Single-leg stance asks the side-hip muscles and tendons to control the pelvis. An irritated outer-hip region may react when that load exceeds its current tolerance.
