Why the exact location matters

Bursae reduce friction where soft tissues pass over bone, but the lower back is not a typical site for the commonly named forms of bursitis. Pain described as low-back bursitis may actually sit over the upper buttock, outer hip, pelvis, or a spinal joint. Tenderness at the surface, pain with lying on one side, pain traveling down a leg, and pain triggered by bending suggest different structures. A clinician can map the painful area and examine the hips, nerves, movement, strength, and spine before assigning a diagnosis.

Managing symptoms while seeking clarity

Stay gently active within tolerable limits and vary positions instead of remaining in bed or sitting for long stretches. Temporarily scale down lifting, twisting, or impact that clearly increases symptoms. Heat or a wrapped cold pack may feel soothing, depending on the problem. Avoid forceful self-massage over a new lump or swollen area. A physiotherapy or medical assessment is useful when the source is uncertain; treatment for a bursa differs from care for nerve irritation, a hip tendon problem, or a spinal condition.

Back-pain warning signs

Seek emergency care for new loss of bladder or bowel control, numbness around the genitals or buttocks, or rapidly worsening weakness in either leg. Prompt assessment is also needed after major trauma, or when back pain comes with fever, feeling severely unwell, a hot red swelling, or unexplained difficulty walking. Arrange a non-urgent appointment if pain persists, repeatedly returns, wakes you, spreads into a leg, or limits ordinary activity. Urgency depends on these warning features, not on whether bursitis seems common or uncommon.

Outer-hip pain can be perceived near the buttock or pelvis and may coexist with back pain. Examination is needed to establish which structure is producing the symptoms.

Would a cortisone injection confirm lower-back bursitis?
