What is ischial bursitis?

The ischial tuberosity is the lower part of the pelvis that bears pressure when you sit. A nearby bursa cushions tissues as they move across this bony point.

Where the pain occurs and what provokes it

Discomfort is usually focused deep in the lower buttock and may be tender when pressed. Firm seats, long periods of sitting, cycling, rowing or repeated hip motion can increase compression and friction around the bursa.

Several problems can mimic this pattern. The hamstring tendon attaches at the same bone, while nerve irritation from the back can send pain into the buttock or leg. A clinician checks whether stretching or resisted movement loads the tendon and whether numbness, weakness or back movement points toward a nerve source.

Reducing irritation while staying active

Break up long sitting periods and choose a supportive cushion that unloads the tender point without pressing its centre. Temporarily scale back cycling, rowing or other activities that reproduce the pain, then rebuild exposure as symptoms settle.

Physiotherapy can address hip and hamstring capacity, posture and gradual return to sport. Cold or warmth may provide comfort according to preference. Medicine or a guided cortisone injection may be considered after a clinician has checked the diagnosis, medication risks and signs of infection.

When buttock pain needs assessment

Seek a review if pain persists despite pressure modification, disturbs sleep, follows a fall, or prevents normal walking and sitting. New tingling, numbness, leg weakness or pain travelling below the buttock can suggest nerve involvement rather than an isolated bursa problem.

Urgent care is needed for loss of bladder or bowel control, numbness around the groin, rapidly progressive weakness, fever with a hot swollen area, or severe pain after substantial trauma.

Ask whether the bursa, hamstring tendon, hip or lower back better matches your examination. Discuss how to adjust seating and exercise, which strengthening work is suitable, and whether imaging would change management.

Can ischial bursitis cause pain down the leg?

Pain may spread around the lower buttock, but prominent tingling, numbness, weakness or pain extending farther down the leg warrants assessment for nerve irritation.

What type of cushion may help?

A firm, supportive pressure-relieving cushion may reduce contact over the sore sitting bone. Very soft seating can allow the area to sink and remain compressed.

Is ischial bursitis the same as proximal hamstring tendinopathy?

No. The bursa and tendon are separate structures beside the same bone. Examination helps identify which tissue is the main pain source.
