Shoulder bursitis: understanding pain and recovery

Short answer
Shoulder bursitis is irritation of a cushioning sac beneath the shoulder structures, often making arm elevation or reaching painful.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Repeated or newly increased overhead loading
- Pain area
- Shoulder and upper outer arm
- Movement approach
- Modify painful load while preserving gentle mobility
The short answer
Pain may be felt over the upper outer arm and can be worse when reaching overhead, fastening clothing behind the back, or lying on that shoulder. The bursa sits close to the rotator cuff tendons, so both tissues can be irritated together and symptoms may overlap.
What the pain pattern can suggest
Repeated overhead work, throwing, lifting, or a sudden increase in upper-body activity may overload the area. Pain during a particular part of raising the arm can fit irritation beneath the shoulder arch, but it does not identify the bursa by itself. Neck-related pain, arthritis, tendon injury, and stiffness of the shoulder capsule can feel similar.
A clinician may compare active and assisted movement, test shoulder strength, and examine the neck. Imaging is usually reserved for situations in which trauma, marked weakness, persistent limitation, or an alternative diagnosis changes what should happen next.
Keep the shoulder moving comfortably
Scale back repeated overhead tasks and heavy lifting for a while, but avoid holding the arm completely still. Gentle movement within a tolerable range helps preserve mobility. When sleeping, support the affected arm on a pillow or lie on the opposite side rather than placing body weight on the sore shoulder.
Physiotherapy can progress shoulder-blade control, rotator cuff capacity, and task-specific loading. Pain medicine or an anti-inflammatory may not suit every person; a clinician or pharmacist can check medical conditions and other medicines. An injection may be discussed when pain is limiting rehabilitation and the likely source has been assessed.
When shoulder symptoms need assessment
Seek urgent care if shoulder pain comes with chest pressure, breathlessness, sweating, faintness, or pain spreading to the jaw or arm. Get prompt assessment for a hot, red, rapidly swollen shoulder with fever, or after an injury that leaves the joint deformed or the arm unusable.
Arrange a clinical review for new marked weakness, persistent night pain unrelated to position, increasing stiffness, numbness, or symptoms that are not settling after modifying activity. Sudden inability to raise the arm after a fall or forceful movement may point to more than uncomplicated bursitis.
Plan your shoulder review
Ask whether the findings suggest bursal irritation, tendon involvement, joint stiffness, or pain referred from the neck. Clarify how much overhead activity is reasonable, which exercises match your current strength and movement, and what would make an injection or scan useful.

Questions people ask
Should I use a sling for shoulder bursitis?
Routine sling use can encourage stiffness and is generally unnecessary unless a clinician recommends it for another injury. Support the arm for comfort while continuing gentle movement.
Can I continue gym training?
You may be able to train around the symptoms by reducing overhead load, range, or resistance. Pain that escalates during or after a session means the programme needs further adjustment.
