Bursitis around the gluteal area: what it may mean

The short answer
Outer-hip bursae can hurt with side-lying, stairs, or walking, while irritation near the sitting bones may be aggravated by sitting on a firm surface. Tendons, the lower back, nerves, and the hip joint can produce pain in similar areas.
Pinpointing the source of buttock pain
The point of greatest tenderness and the movement that reproduces symptoms help distinguish between nearby structures. Pain directly over the outer hip differs from deep buttock pain that travels down the leg or is associated with tingling. Discomfort close to the sitting bone may also come from the hamstring attachment rather than a bursa.
Assessment may include hip and spine movement, leg strength, tendon loading, sensation, and walking. Scans are sometimes used when symptoms followed trauma, the examination is unclear, or treatment aimed at the suspected structure has not helped.
Change pressure without stopping movement
If sitting triggers pain, break up long periods and use a supportive surface that reduces direct pressure without forcing an awkward posture. For outer-hip symptoms, avoid lying on the painful side and consider a pillow between the knees. Keep up comfortable daily movement while temporarily reducing hills, deep bending, or other clear triggers.
Rehabilitation may address hip and trunk strength, movement control, and a gradual return to normal loading. Avoid forceful stretching or massage directly over an acutely tender area if it makes pain flare. Discuss medication or an injection with a clinician who has considered the exact pain source and your wider health.
Red flags with buttock pain
Seek urgent care for new loss of bladder or bowel control, numbness around the groin or seat area, or rapidly worsening weakness in a leg. These are neurological warning signs rather than a typical bursa problem.
A hot, red swelling with fever or feeling unwell also needs prompt assessment. After a significant fall, get medical help if weight-bearing is not possible. Arrange a routine review if persistent pain disturbs sleep, repeatedly limits walking or sitting, or spreads down the leg with numbness or tingling.
Clarify what is generating the pain
Ask whether the examination points to a bursa, tendon, nerve, joint, or referred back pain. Discuss which positions should be modified, which exercises fit the suspected tissue, what improvement would be expected, and whether imaging would change the treatment plan.

Is gluteal bursitis the same as sciatica?
No. Sciatica describes nerve-related symptoms and may include pain, tingling, numbness, or weakness down the leg. A bursa more often causes local pressure-sensitive tenderness.
Will stretching the glutes help?
Stretching is not automatically helpful and may compress an irritated structure. Choose movements based on the pain location and stop any exercise that clearly worsens symptoms.
