What should you know about bursitis hip?

Short answer

Hip bursitis is irritation around a fluid-filled bursa, often causing tenderness and pain at the outer hip.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical location
Tenderness over the outer side of the hip
Common aggravators
Side-lying, stairs, prolonged walking, and sudden increases in activity
Assessment
Usually begins with symptom history and examination of the hip, back, walking, and strength

What hip bursitis means

Bursae are small cushioning sacs that reduce friction where tendons move near bone. Around the hip, irritation commonly produces pain over the bony point at the side of the thigh. The area may hurt when pressed, when lying on that side, while climbing stairs, or after a change in walking or exercise load. Pain can spread down the outer thigh, but numbness or tingling suggests that a nerve may also need assessment. Similar symptoms can arise from nearby tendons, the hip joint, or the lower back, so the location of pain alone does not confirm bursitis.

What you can do

Temporarily reduce activities that repeatedly provoke the pain rather than stopping all movement. Avoid sleeping directly on the sore side; a pillow between the knees can reduce compression when lying on the other side. A cold pack wrapped in cloth may settle soreness after activity. Gentle walking and a gradual return to loading are often easier to tolerate than sudden hills, long distances, or repeated side-to-side exercise. A clinician or physiotherapist can check hip strength, walking pattern, tendon involvement, and whether exercises should be adjusted. Anti-inflammatory medicine is not suitable for everyone, particularly people with stomach, kidney, bleeding, heart, pregnancy, or medication concerns, so check with a clinician or pharmacist before using it.

When to seek medical care

Arrange an assessment if pain is persistent, keeps returning, disrupts sleep, limits walking, or followed a fall. Seek urgent care for severe pain with an inability to bear weight, a visibly deformed hip, rapidly increasing redness or warmth, fever with a hot swollen area, or new weakness or loss of feeling in the leg. Sudden leg swelling with chest pain or breathlessness also needs emergency evaluation. These features can point to an injury, infection, clot, or another problem that should not be managed as routine bursitis.

Questions for your clinician

Ask which structure appears to be generating the pain, whether tendon or joint problems need consideration, which movements are safe to continue, and how to progress strengthening without repeatedly flaring symptoms. If an injection is discussed, ask what diagnosis it is intended to treat, what short-term precautions apply, and what the next step would be if relief is limited.

Questions people ask

Can I exercise with hip bursitis?

Often, yes, if the activity is modified so pain remains manageable and does not flare afterward. Lower the load or range, avoid direct pressure on the sore side, and build back gradually.

Does hip bursitis always cause visible swelling?

No. The irritated structures can be deep enough that pain and tenderness are more noticeable than swelling. Obvious warmth, redness, or rapidly expanding swelling warrants medical assessment.

When might an injection be considered?

A clinician may discuss a cortisone injection after confirming the likely pain source and reviewing other treatment options, health conditions, medicines, and the expected benefits and risks.
