An accurate assessment matters because the muscle itself, its tendon attachment, the hip joint, and the lower back require different management. Treatment often pairs avoidance of strongly compressive positions with progressive strengthening for the hip abductors.

The relationship between muscle, tendon, and bursa

The gluteus medius helps keep the pelvis level while you stand on one leg. Its tendon attaches at the outer upper thigh bone, close to bursae that reduce friction. Repeated loading or compression can make this region tender during walking, stairs, running, or side-lying.

Weakness during an examination does not automatically mean a torn muscle; pain can temporarily inhibit force. A clinician combines the history, tender location, resisted movements, walking pattern, and sometimes imaging to refine the diagnosis.

Protect the area, then restore strength

Reduce positions that squeeze the outer hip, such as lying on that side, sitting with crossed legs, or standing with body weight dropped onto one hip. Adjust repeated hills, stairs, or long walks to a tolerable level while retaining ordinary movement that does not provoke a flare.

Strength work should advance in response to symptoms, beginning with a level you can control and progressing toward single-leg tasks needed for daily life or sport. Aggressive stretching across the body may compress the painful attachment and is not always helpful. A physiotherapist can select exercises after checking pelvic control and other contributing joints.

When an examination is needed

Consult a clinician if the pain steadily worsens, produces a persistent limp, repeatedly interrupts sleep, or follows a fall. Immediate assessment is warranted if you cannot bear weight, develop fever with a hot or red hip area, notice new leg weakness or numbness, or lose bladder or bowel control.
