Understanding pain on both outer hips

The bursae near the greater trochanter help tissues glide over the prominent bone at the side of the hip. In this region, bursal irritation and gluteal tendon pain can overlap. Symptoms may be tender to touch and worse when lying on either side, standing on one leg, climbing stairs or walking farther than usual.

Pain on both sides may reflect similar loading of each hip, a recent activity change or compensation for another problem. Pain from the lower back, hip joints or systemic inflammation can mimic the pattern. A physical examination helps establish whether tenderness and resisted movements point to the outer-hip tissues.

Reduce compression while keeping active

Avoid sleeping directly on the painful side; when side-lying, place a supportive pillow between the knees, or try lying on your back. Do not stand with one hip dropped outward or sit with the legs crossed if those positions provoke pain. Temporarily shorten walks and reduce repeated stairs without stopping all movement.

A physiotherapy plan may progressively strengthen the hip and trunk while adjusting loads that keep the area irritable. Because both sides hurt, exercise selection and sleeping positions need particular care. Anti-inflammatory medicine is an option only when it is appropriate for your medical history and current medicines.

When bilateral hip pain needs review

Obtain urgent help after a fall if you cannot stand or bear weight, or if a hip looks shortened or deformed. Fever with a hot, swollen joint, rapidly worsening pain, new leg weakness, numbness around the groin, or loss of bladder or bowel control also needs immediate assessment.

See a clinician if pain steadily worsens, disrupts sleep despite position changes, causes a persistent limp, or occurs with marked morning stiffness or illness elsewhere. Assessment is also useful when self-care has not restored ordinary walking and stair use.

Plan for both sides

Ask whether the bursae, gluteal tendons, hip joints or lower back better explain the pattern. Clarify how to dose walking, which sleeping position protects both hips, whether each side needs the same exercises, and what progress markers should guide activity increases.

Can bursitis affect both hips at once?

Yes, tissues on both outer hips can become irritated. However, bilateral pain has several possible sources, so the distribution alone does not confirm bursitis.

Should I stop walking when both hips hurt?

Complete rest can reduce capacity. Instead, choose a distance and pace that do not cause a marked symptom increase, then progress gradually as comfort and strength improve.

Why does lying on my side hurt?
