Direct answer
The iliotibial band, often called the IT band, is a strong strip of connective tissue along the outside of the thigh. Nearby bursae reduce friction between moving tissues and bone. Pain in this region may involve a bursa, the IT band, surrounding tendons, or more than one structure.
Where the discomfort comes from
At the hip, symptoms may be tender over the bony point on the outer side and aggravated by lying on that side, climbing stairs, or prolonged walking. Near the knee, IT band irritation is often felt at the outer knee during repetitive flexion and extension. These patterns guide an examination but do not establish the exact tissue involved.
A clinician will usually check hip and knee motion, muscle strength, walking or running mechanics, spinal and nerve signs, and the precise area of tenderness. Imaging may be considered when the diagnosis is unclear, symptoms persist, or another condition needs to be assessed.
Settling irritation and rebuilding capacity
Temporarily reduce the activity that reliably triggers pain, but keep comfortable movement in your day. Adjusting running distance, hills, cycling resistance, sitting position, or direct pressure on the outer hip can reduce irritation. A pillow between the knees may make side-lying more comfortable.
Rehabilitation often targets hip strength, control of the pelvis and knee, and a gradual return to the provoking activity. Forceful rolling directly over a very tender bursa can worsen symptoms. Anti-inflammatory medication or an injection may be considered after checking your health history; neither replaces correction of a persistent loading problem.
Signs that need medical review
Get urgent assessment after a major fall if you cannot stand or walk, the limb looks deformed, or pain is severe. A hot, red, rapidly enlarging swelling with fever or feeling unwell also requires prompt care because infection must be considered.
Book an appointment if pain repeatedly interrupts sleep, causes a persistent limp, includes numbness or weakness, or fails to improve after sensible activity modification. Sudden calf swelling, chest pain, or breathlessness warrants urgent medical help rather than exercise-based care.

Should I stretch the IT band if I have bursitis?
Gentle mobility may feel comfortable, but aggressive stretching that compresses a tender outer hip can aggravate symptoms. A physiotherapist can select movements based on the painful site and your examination.
Is foam rolling helpful for outer thigh pain?
Some people tolerate rolling the surrounding muscles, but pressing hard over an inflamed bursa may increase pain. Stop if soreness lingers or symptoms intensify.
Can I keep running with IT band or bursal pain?
