Bursitis and Gout: How They Differ

Bursitis is inflammation of a cushioning sac near a joint. Gout is an inflammatory arthritis caused by urate crystals within a joint; crystals can also irritate nearby bursae. Location, examination, health history, and sometimes fluid testing help separate these possibilities from infection.

Bursal pain is often most tender directly over a bursa, such as at the point of the elbow or front of the kneecap, and pressure on that spot may be especially uncomfortable. Gout commonly produces intense joint tenderness with warmth, swelling, and limited movement. Symptoms alone can overlap, so appearance is not enough to confirm the cause.

A clinician may ask about previous attacks, kidney problems, medicines, skin breaks, recent injury, fever, and which tissue seems swollen. Joint or bursal aspiration can obtain fluid for crystal analysis and infection testing when the diagnosis remains uncertain or infection is a concern.

Rest the painful area from pressure and repeated loading, elevate it if practical, and use a wrapped cold pack for short periods. Do not puncture or squeeze a swollen bursa. If you already have a gout plan, follow the prescribed instructions rather than borrowing another person's medicine or changing doses yourself.

Treatment may include anti-inflammatory medication when safe for you. Aspiration, a cortisone injection, antibiotics, or longer-term urate-lowering treatment serves a different purpose and should follow clinical assessment. Tell the prescriber about kidney disease, stomach ulcers, blood-thinning medicines, pregnancy, and medication allergies.

Seek same-day medical assessment for a hot, red, rapidly swelling joint or bursa, particularly with fever, chills, feeling unwell, a recent wound, or reduced immunity. Infection can resemble either gout or non-infectious bursitis and needs prompt evaluation.

Urgent help is also appropriate if you cannot bear weight, cannot use the limb, or the pain followed a significant injury. Arrange a routine review for repeated attacks, a swelling that persists, or symptoms that do not respond to the plan you were given.
