What Should You Know About a Burn Blister Scar?

A blister forms when heat damages the skin and fluid collects between skin layers. If the injury extends beyond the most superficial layer, the body repairs the area with new tissue that may differ in colour, thickness, flexibility, or texture. A scar can be flat, indented, raised, tight, itchy, or tender. Its appearance may change as the skin matures.

The look of a scar does not show its full depth. A clinician may need to examine the burn, its location, how it is healing, and whether movement is restricted. Burns involving the face, hands, feet, genitals, major joints, or a large area deserve particular care because healing can affect function as well as appearance.

For a fresh burn, cool the area with cool running water and remove nearby jewellery or clothing that is not stuck to the skin. Do not apply ice, burst the blister, peel loose skin, or use household creams on an open burn. Cover it with a clean, non-stick dressing and arrange medical advice when the injury is deep, extensive, contaminated, or difficult to judge.

Once the wound has fully closed, protect the area from sun exposure and keep the skin moisturised if advised. Do not massage, stretch, or apply silicone products until a clinician confirms that the surface is ready. Persistent tightness, itch, pain, or restricted movement can justify referral to a scar specialist. Options may include Fractional Laser Treatment, Scar Revision Surgery, or a Skin Graft, depending on the scar and the surrounding tissue.

Seek urgent assessment for a burn that is deep, white or charred, rapidly swelling, circumferential, caused by electricity or chemicals, or located over a major joint or sensitive body area. Get prompt care if the wound becomes increasingly red, warm, swollen, painful, or foul-smelling, or if pus, fever, or red streaking develops. Difficulty moving the area, numbness, or a scar that is tightening also needs medical review.

Ask how deep the burn appears, whether the blister should remain intact, and what dressing is suitable. Ask when the wound is ready for moisturising, sun protection, silicone, or massage; how to monitor movement; and whether a scar service or reconstructive treatment could help if the scar stays raised, painful, itchy, or tight.
